The form includes a printed bilingual text, in Portuguese and English, with blank spaces to be filled. The first page, surmounted by a large illustrated sticker, intaglio engraved with the coat of arms of Portugal, begins: Francisco de Sousa Maximiliano de Sousa ... [Secretary of the Regency of the Kingdom, 1821] Do make known to whom this Passport seeing... On the back: this Passport (No.) authorises the Ship herein enounced, to take on board once any number of Slaves, not to exceed () being five per two tons. The text is repeated in the English language on the second Folio. The handwritten document contains, apparently, the instructions for filling in the Passport, showing, in the end, the indication: Castlereagh (L.S.); Count of Palmella (L.S.). Notes: accompanied by handwritten minute
